Carver County Public Health is seeking community input. Visit the Carver County website to take a short survey that allows respondents to rank what they believe are the top ten health issues in Carver County.
Every five years, local public health departments in Minnesota work with partners to identify health issues facing their communities. This includes collecting input from residents and individuals working in the community. Carver County Public Health will use this input in developing the department work plan for the next five years. This is a chance to offer your own views.
Visit http://www.co.carver.mn.us/ to take the survey. It takes about five minutes to complete and it will be available through Sunday, June 15. If you have any questions about the survey, or would like to receive a paper copy of the survey in the mail, contact Carver County Public Health at (952) 361-1329. When the five-year work plan is assembled, a summary will be posted on the department’s website.
